Level 1 Model building Lesson 4 Lists

So I am playing catch up and I cant figure out the correct mapping for the list. I am on 4.6.5 Activity Import Data into G3 Location List. It doesn't outline the file mapping and sources like it did in the previous activity so Im just wondering what the correct data is for that. I import the list but then I get stuck on verifying the file options and mapping
