How to get access to old Model

Hello,
I have completed Level 2 by completing all 4 exams(Sprint1,2,3,Conclusion) 6 months back but did a mistake by not submitting the model history as I have assumed by finishing the 4 exams I can start Level 3 later. But now I am unable to start Level 3 as I have not submitted model history of (Supply Chain model) but to submit the model history I do not have access to the (supply Chain) model, May I know how to get access to that model now.
